-Original Birth Certificate, Photocopy of the original birth certificate (duplicate)
-undergo headstart school and division level assessments
Grade I entrants:
-Original Birth Certificate, Photocopy of the original birth certificate (duplicate)
-undergo School Readiness Assessment (SReA)
Grade II-VI:
-Reports Cards (Form 178)
Transferees:
-Report Cards (Form 178)
-Permanent Record Form (Form 137E) to be requested from the school the transferee last attended
